The ICH Score (Intracerebral Hemorrhage Score) is a simple and widely validated clinical grading scale used to predict 30-day mortality in patients presenting with spontaneous intracerebral hemorrhage (ICH). It was developed to provide rapid risk stratification at the patient's bedside, based on key factors readily available upon initial evaluation, particularly from the initial non-contrast head CT scan. The score is calculated by summing points assigned to five components: (1) Glasgow Coma Scale (GCS) Score: 2 points for GCS 3-4, 1 point for GCS 5-12 (0 points for GCS 13-15); (2) ICH Volume: 1 point if the hemorrhage volume is 30 mL or greater (0 points if < 30 mL); (3) Intraventricular Hemorrhage (IVH): 1 point if there is blood extension into the ventricular system (0 points if absent); (4) Infratentorial Origin: 1 point if the hemorrhage originates in the brainstem or cerebellum (i.e., below the tentorium cerebelli) (0 points for supratentorial/lobar/ganglionic location); and (5) Age: 1 point if the patient is 80 years or older (0 points if < 80 years). The total score ranges from 0 to 6. A higher score directly correlates with a worse prognosis, with mortality rates increasing significantly with each additional point; a score of 0 predicts a 30-day mortality rate near zero or very low, while a score of 5 or 6 is associated with extremely high mortality, often exceeding 90-100%.
