The HAS-BLED score is a risk assessment tool used to estimate the 1-year risk of major bleeding in patients with atrial fibrillation (AF) who are taking oral anticoagulants. It is a clinical mnemonic that helps physicians identify modifiable and non-modifiable bleeding risk factors. The score includes: Hypertension (uncontrolled blood pressure), Abnormal renal or liver function (one point for each), Stroke (previous stroke), Bleeding (history of major bleeding or predisposition), Labile INRs (labile International Normalized Ratios, for patients on warfarin), Elderly (e.g., >65 years), and Drugs or alcohol (concomitant use of drugs that increase bleeding risk, like NSAIDs, or excessive alcohol consumption). A higher score indicates a higher risk of major bleeding.

Formula

Calculation Methodology The HAS-BLED score is calculated by assigning 1 point for each risk factor: Hypertension (SBP > 160 mmHg), abnormal Renal or Liver function (1 point each), previous Stroke, Bleeding history or predisposition, labile INR, Elderly (> 65 years), and Drugs or Alcohol use (1 point each). Scores range from 0 to 9, with ≥ 3 indicating a high risk of major bleeding.
