    The SOFA Score (Sequential Organ Failure Assessment) is a fundamental tool in intensive care used to track the status of a patient's organ function or rate of failure during their ICU stay. Primarily used in cases of sepsis, it evaluates six organ systems: respiratory, cardiovascular, hepatic, coagulation, renal, and neurological. Each system is scored from 0 to 4, with a total score ranging up to 24. Higher scores correlate with increased mortality risk. The clinical value of the SOFA score lies in its ability to monitor disease progression and the effectiveness of medical interventions over time, rather than providing a single static diagnosis.
